We Need Your Help!!!!!

Dear Participating Chef,
     Below is a form asking for the typed information we will need to create The SDHS Cookbook. You will notice photo (e-mailed or on cd), favorite something and a phone number or e-mail addresses are optional. We thought these personal touches would add additional interest to our keepsake cookbook. The information on the submission form is what would be included in the cookbook.
     However,  we still might need to contact you if we have questions about your recipe. So we are asking for your phone number or e-mail address for our purposes only. The space for that information is at the very bottom of the submission form. Be assured your phone number or e-mail information will not be distributed with your recipe unless you have filled out that portion of the submission form.
